Re: Mr Sean William,aged 72


I am referring Mr Sean Williams, who has been diagnosed with chronic obstructive pulmonary disease, requires appropriate advice and instructions regarding proper usage of inhaler and spacer from your service.


Upon initial visit on 06 February 2019, Mr William complained of increased shortness of breath especially while performing activities and reported the ineffectiveness of salbutamol. On examination, good air entry on both lungs were detected whereas, advises regarding appropriate use of inhaler was given as faulty technique was suspected.


On today's visit, Mr William complained of persisting symptoms while exertion. Discussions about the techniques in using inhaler with spacer were made;however, he was unconvinced and claimed that this is not the way and dose my pharmacist taught. Currently, he is on ventolin HFA puffs under supervision.


Mr William, a widower has two children and lives alone. He is on daily dressing of his leg ulcer as well as he has persisting functional difficulties.


Pertaining to the above details, it is profoundly beneficial if you could provide necessary suggestions and information regarding the proper use of inhaler and spacer to Mr William.
