Re: Ms Eiden McGrath, DOB:16 August 1965


Thank you for accepting Ms Eiden McGrath, who was diagnosed with type 2 diabetes mellitus. She requires surgical debridement for the foot ulcer from your facility.


Today, Ms McGrath was presented with the history of foot ulcer since last two weeks as well as complaints of tiredness and lethargy. On examination, tenderness and swelling were noted. Blood tests were performed, which revealed elevated blood sugar levels. Consequently, type 2 diabetes mellitus, osteomyelitis and grade 2 wound were ruled out. Fortamet 1000 mg once daily, Albactin 500 mg for 10 days and Tapcynta 100 mg twice daily in the morning were prescribed.


In addition, wound dressing was applied and walking stick was provided. Use of well - cushioned footwear has been recommended. Apart from that, explanation on replacing Tapcynta with Duovolt 50 mg 1 week later has been provided. Advice to reduce alcohol consumption and avoid heavy lifting has been provided. Importance of healthy diet and having proper bed rest were discussed.


She has had hypertension since 2009 and has been non - compliant to atenolol 50 mg. She had ORIF hysterectomy in 2010 and 2012 respectively. Head injury in 2014 and use of over-the-counter, pheniramine maleate for occasional rhinitis were noted. Allergy with dust, pollen and shellfish was reported.


Based on the aforementioned circumstances, possible debridement of foot ulcer would be profoundly beneficial. Follow-up appointment after one week has been scheduled at our facility.
