Re: Mr Peter Hendrick, DOB: 21 May 1972
Mr Peter hendrick, who is recuperating from myocardial infarction, requires home visit from your service to provide assistance in smooth transition back to normal life following his discharge.
On 2nd July 2019, he was presented with signs and symptoms indicative of myocardial infarction. Therefore, a successful emergency angioplasty was performed. Cardiac exercise program was initiated by the Physiotherapist. Diet for reducing the cholesterol and body weight was recommended by the Dietitian. Currently, he has attained an excellent progress and ready for the discharge.
Mr Hendrick works as a truck driver in mixed shifts and lives with his wife Beula, who is a school teacher. His food preferences are fries, hamburgers, doughnuts and ice cream. Based on the aforementioned circumstances, advice on returning to work, abstain driving for next 6 weeks and assistance in daily activities would be profoundly appreciated.
