Re:Betty Olsen, DOB: 29 January 1926
I am referring Ms Betty Olsen, who shows signs and symptoms of cardiac problem. She requires urgent assessment and management.
Today, Ms Olsen complains of tiredness and weakness in the morning and blood pressure was 180/95. The symptoms have been continued with nauseous feeling along with shoulder and neck pain, for which 1 Normison and 2 Panadol have been commenced and BP was 175/95. In addition, symptoms have been got worst at night with persistent chest pain, distressed, pale and sweaty.
Recently, she has been experiencing indigestion which was settled down with Mayalta, as well as insomnia and shoulder pain.
Pertaining to medical history, she has had hypertension and hypothyroidism since 1977, 2003 respectively. In addition, she is allergic to penicillin.
