Re:Mr Peter Hendrick,DOB:21 May 1972


I am writing to refer Mr Peter Hendrick who requires home visit for instruction on how to manage his daily activity independtly after discharge.He was diagnosed with myocardial infraction.


On 2 July 2019,Mr Hendrick presented to our hospital with constant discomfort on chest.Followed by emergency angioplasty done.In addition to that, his complete blood count indicates high cholesterol level.He is having the family history of cardiac disease.After the surgery he got reviewed by the physiotherapist and advices cardiac exercised program.Moreover his body mass index indicates 29 due to that dietician provided a diet plan to reduce his weight and cholesterol level.


Mr Hendrick works as truck driver,who leaves with his wife and family.Upon discharge, he is commenced with certain medication.He is advised to not to drive for six weeks.Along with that,he is educated regarding the reoccurance of cardiac diseases and his risk factor.


It would be greatly appreciated, if you provide a home visit for Mr Hendrick to provide advice on returning to work and to maintain his normal daily activities.
