Dear Ms Beckinsale,


Re: Mr Sean William, aged 72


I am writing to update the current status of Mr Sean William, who has been suffering from shortness of breath possibly due to chronic obstructive pulmonary disease. She requires your guidance for inhaler usage.


On 6 February 2019, on my vesit, Mrs William complained about increased shortness of breath in exertion especially at day time. On assessment, he Wing to inappropriate used of sabbutamol inhaler and/or exercise induced bronchospasm.


Therefore, stopped sabbutamol and ipratropium and commenced on ventilon HFA 2 puffs under supervision. On subsequent days,she has been experiencing the same symptom and reported that recently prescribed inhaler was ineffective.


Thus, Mr Willam was educated about the inhaler use with spacer. However,she refused that his pharmasist thought him in an different way.


Pertaining to the above mentioned information,it would be profoundly beneficial if you could teach Ms Williams regarding the inhaler use for his will being and speedy recovery.


Hereby,I have enclosed Mr Williams medical history for your perusal.
