Re:Mr Gerald Baker, aged 79
Dear Ms Bruin,
Iam writing to update the current health status of Mr Baker, who is recuperating from left total hip replacement surgery. He requires ongoing care and assistance after beidischarged back into your care.
On 3 September 2010, Mr Baker was admitted for the above mentioned surgery. Post operatively, he was disoriented possibly related to anaesthesia effect. In addition, Hb was 72 for which 3 units of red blood has been transfused. Besides this, daily dressing has been done, assisted him in stretching and strengthening exercises and wheelie walker was provided. As a result, good mobility has been noticed post-operatively.
Upon discharge, exercise program have to be continued, walker and wedge pillow will be provided by the hospital as well as this, a toilet raiser on hire will be arranged by the social worker for 2 weeks. So, it would be greatly appreciated if you could monitor his blood levels and medications compliance. In addition, staples to be removed on 9 September and arrange a follow up with FBE and UEC results at City Hospital Clinic.
