RE: Ms Patricia Styles, DOB: 27/04/1957
I am writing to refer Ms Styles who was managed for pericarditis and has developed complications. She would need your urgent assessment and treatment.
Ms Style was visited at home today as a routine follow-up management after she was successfully treated for pericarditis. However, she was unhappy and reported that she has been experiencing a chest pain, shortness of breath and fatigue. Her vital signs include: T 38.1, RR 28, HR 115 and BP 125/78. Please note that she has been taking her medications as prescribed.
Initially on 23/08/2019, Ms Style presented with similar signs and symptoms. A throat swab and an echocardiogram were done which confirmed type B viral influenza pericarditis. She was then admitted and treated with fluid and analgesics. She was subsequently discharged home on medications.
It is worth mentioning that Ms Style has had hypertension, diabetes and depression since 2011, 2013 and 2016 respectively. She is on quinapril, metformin and glicazide.
In view of the above, I would apply you could urgently review and manage Ms Style. Kindly update me on her progress.
