Dear Nurse,

Re:Mr Andrew O'Connor, aged 74

Mr Andrew O'Connor, who is recuperating from a stroke, requires home visits for monitoring and continued care following his discharge on 22 March 2010

On 02 December 2009,Mr O'Connor was admitted and diagnosed with cerebro vascular accident. Subsequently, right side hemiplegia was noted;therefore, immediate physiotherapy and occupational therapy were initiated. Assistance in activities of daily living has been required. Presently, he is mobilise with frame and start feeding himself. In addition, mood swings was reported. A family meeting was held on 09 March 2010 and decided to send home with modifications assessed and approved by occupational therapist.

Mr O'Connor is married and lives with wife a 71-year-old Mary, who has angina.

As per discharge plan, it would be appreciated if you could provide home visit twice daily to assist Mr O'Connor for daily living activities and monitoring his medication compliance. Kindly,supervise his wife health conditon and please refer to hospital for any emergency.
