Re: Ms Olivia Jack, aged 19
Ms Olivia Jack, who delivered a female baby via cesarean section requires home visit to educate and assist her in caring the baby, from your service, following her discharge today.
She is single and lives alone in a rental shared apartment at flat 7,9 Riverside Southbank Melbourne Victoria 3006 Australia. Her pregnancy was unwanted and has no contact with the father of baby or with family members. She wishes to offer the baby for adoption.
On 9 June 2019, she presented at 1600 hours with contractions lasting for 10 minutes. Emergency cesarean section was performed due to fetal distress and failure to progress. The baby was delivered on 10 june 2019 at 03:12 hours with weight of 3.9 kg. Postpartum hemorrhage 800ml was noted, which has reduced at present. Wound was cleaned, dried and dressing was applied. Iron and vitamin C supplements were prescribed. Breastfeeding was started. Bottle feed was preferred due to lack of confidence in breastfeeding. Currently, the weight of baby is 3.7 kg.
Based on the aforementioned circumstances, education for breastfeeding and bathing technique for baby would be profoundly beneficial.
