Dear Nurse,
Re: Mrs Monica Osburn, aged 69
Mrs Monica Osburn, a - known - case of hypertension and depression is being referred into your service for further care and assistance.
She is a widow and lives alone. Her daughter resides near to Woodville and has requested her mother to shift there so that she can look after her. Apart from that, she is hypertensive since last 10 years and has had depression for 2 years, for which she takes anti - hypertensives and anti - depressants. She has poor - compliance to the medication. When she gets depressive episodes she consumes excessive alcohol.
Since September 2003, regular monitoring for medication compliance and alcohol intake has been provided to her, which should be continued.
Based on the aforementioned circumstances, meeting with the medical practitioner is advised to to her. Healthy and balanced diet would be beneficial. Liase with elderly citizens' club, voluntary groups and family involvement should be recommended to treat depression.
