Dear Sir/Madam,
Re: Ms Diane Carpenter,aged 58
I am writing to request Ms Diane Carpenter requires ongoing care and assistance, who underwent lung resection. She is being discharged today.
Ms Carpenter was admitted to the hospital on 10 May 2009 and her investigations revealed small patches in her left lung. During hospitalisation, she was also diagnosed with pneumonia. She was stabilised with mechanical ventilation, oxygen support and antibiotic therapy along with physiotherapy and nutritional modifications. However, she has slow mobility due to reluctance to walk and coping difficulties.
Ms Carpenter has recently migrated from Canada and lives in a small rented unit. She could not work due to visa problems and she is being financially assisted by her daughters. She had undergone a mastectomy for breast cancer in 1986. In addition, she has been taking tamoxifen, which is showing good results.
It would be greatly appreciated if you could monitor her medications regimen and continue physiotherapy as well as encourage her to ambulate. It is necessary to arrange psychiatrist visits with her daughters to improve her coping strategies and emotional support. She has an appointment for chemotherapy on 06 June 2009 at Flinders Medical Hospital.
Yours faithfully,
Registered Nurse
