Dear Community Nurse,

Re: Mrs Monica Osburn, aged 60 years

I am writing to refer Mrs Monica Osburn,who requires continuing care and monitoring from your services. She was diagnosed with hypertension and depression. She will be discharged today.

Mrs Osburn has been treated for hypertension over ten years and depression over two years. She is commenced with antihypertensive and antidepressant

Presently, Mrs Osburn lives along in a rented house and planning to move to rented one bedroom unit in woodville which is close to her daughter. She consumes alcohol and it will be more when she got anxious. She have some periodic problems with the self administration of medication.

It would be appreciated, if you could monitor her medication compliance with alcohol intake and diet regime. Kindly encourage her to contact with her daughter and other family members which help her to reduce incidence of derp depression naff anxiety. Moreover, after the discharge and moving to your area kindly inform to her general pratictioner.
