Dear Ms Chuang,

Re: Mr David Felix, aged 23

Thank you for accepting Mr David Felix, who has been recuperating from fractured neck and pelvis injury following a car accident three months ago, into your care.

He is single and his 3rd year student of architect at Adelaide University. He lives alone. His parents wanted to care him; however, they had to return back.

He was admitted on 6 April 2007 to our hospital. He gained neurological damage affecting his mobility, speech and memory due to the accident injuries. He was treated conservatively. Daily physiotherapy, hydrotherapy twice a week and speech therapy thrice a week were initiated. He got bedsores, which were improved by increased mobility. Nurofen 200 mg 4 times a day was given for frequent headaches.

Currently, he has attained a good progress. He mobilizes with the help of wheelchair. Please note, he is now eligible for disability pension.

Given the above, it would be greatly appreciated if you could provide further rehabilitative care and assessment to him. The University should be contacted for continuation of his studies externally. Worthy to note, community access with same age group will be needed to treat depression.

Should you have further queries, please do not hesitate to contact me.

Your sincerely,

Registered Nurse

Royal Adelaide Hospital
