Dear Director,

RE: Akita Annaisha, DOB: 23/09/2014

I am writing regarding Akita, who was admitted to our hospital for acute meningoencephalitis as a result of a complication following mumps. Due to making excellent progress, she is scheduled to be discharged today in your child health service and now requires your continued care and management.

It should be noted that both parents need interpreters because they got a limited understanding of English, however, they understand English moderately. According to the parents, both children have been vaccinated at birth but they do not have access to the record. The parents are unaware of the MMR vaccine for mumps.

It would be greatly appreciated if you could monitor the compliance of the vaccinations schedule and kindly advice them on recommended vaccines for both children. In addition, they will need neurological check-ups for the next three months.
