QQ-3331 Mr Ted Watson - Nursing – Kavita Baudh – Schizophrenia
District Nurse Centre Heidenberg
12 August 2018
Dear District Nurse Supervisor,
Re: Mr Ted Watson, aged 72
Mr Ted Watson, who underwent a right neck of femur repair on 1 May 2018 in Royal Melbourne Hospital, is being referred into your care.
He was transferred into our care on 10 May 2018 with decreased mobility following the surgery. He got febrile episodes due to urinary tract infection, which was treated with ciplox, ural and paracetamol. In addition, he got a large arterial ulcer on right ankle; hance, duoderm dressings were done on weekly basis. Pick up frame was used for mobilization. Assistance with showering and dressing was provided; however, he self cared for indwelling catheter.
He is a widower and lives alone. His daughter, Margrest, who is very supportive, visits him frequently. She can be contacted at 94841123. He attends Ivanhoo Day Centre twice a week and receives home support from Heidelberg Council. Apart from that, he is allergic to Amoxicillin and Penicillin.
Currently, he is medically stable and ready for discharge. He is anxious about managing the things after returning home.
Given the above, it would be greatly appreciated if you could continue all the home supports and assist him with daily activities. Please note, wound dressings should be continued and urinary catheter should be changed after 6 weeks.
Should you have further queries, please do not hesitate to contact me.
Yours faithfully,
