Re : Ms Patricia Styles, aged 63

Ms Patricia Styles , who is suspected to have relapse or complication ofpericarditis, requires an urgent assessment and further management from your facility.

Ms Patricia Styles, was admitted in the Green Valley Hospital on 7Aprile 2018 with diagnosis of Type Binfluenza Plus pericarditis.

Forwhich She was treated with antibiotics andIntra venious saline. Consequently , She was discharged on 9 April 2018, following which home visits were advised. She was on self home care after discharge, and doing well adays ago after her discharge.

Upon 2nd home visit on 14 Aprile 2018, Ms styles had complaints of Chest pain, Shortness of breath and fatigue on plysical examination, febile 382, tachycardia, la2 HR, and hy pertension , 1 80 / 90 multy ware reported .

Ms Styles, who lives alone, is helpled by her cousin occussionally. She has haddiabetes mellitus, hypertension and depression sind 2009, 2012 and 2015 respectively. There fore Jab Capire 1 6. 25 M twice a day, Metformin, sony, twice a day and Glipizide long once daily have been Commenced ..
