Re: Mrs Betty Olsen, D.O.B:29/01/1926


Mrs Olsen, who is a resident of Golden Pond Retirement Village is transferred for immediate admission and assessment of her condition as all of a sudden she had complaints of persistent chest pain, distress and increase in blood pressure.

Since two three days,she had symptoms of shoulder pain, indigestion for which he regular use of medications Panadol and Mylanta use to relieve ;however from today morning she looked very tired , weak and confined to bed.


Today May 09pm , inspite of taking her usual medications, she felt nauseous and had difficulty in sleeping with shoulder and neck pain along with increased blood pressure. At 10.45 pm during recheck blood pressure raised to 190/ 100 along with other symptoms like chest pain, sweating and looking pale and distress; therefore immediately shifted to hospital.


Mrs Olsen , has hyperthyroidism, hypertension and Glaucoma since many years. Her medications include Karvea 150mg, Oroxine 0.1 mg once daily along with eye drops daily two times and norm is on 10 mg if necessary.


She is normally alert and oriented, with good mobilization using walking stick even though she has arthritis in hands and uses continence pads .


Based on the above mentioned details, it is greatly appreciated if you could admit and provide further management.
