Re: Mr Peter Dunbar, aged 87 years
Mr Peter Dunbar, who was non compliant with diabetic management. He requires health education and monitoring as he is moving to your area with his daughter.
Mr Dunbar has had diabetic mellitus type two since 2016. He was managed with dietary guidelines;however, following his wife demise he was depressed and non adhere with diet and exrecises. Subsequent visits, he was recommended to take tablet metformin; although, he blames poor memory and he appears unmotivated.
In October 2018 Mr Dunbar presented with stick, probably due to diabetic neuropathy. He was not comply with diabetic management ;therefore, he was disscussed regarding community based delivery service and retirement village.
Mr Dunbar has hypertension and he takes ramipril 5mg daily. He had an episode of myocardial infarction in 2018 and post MI he was diagnosed with atrial fibrilation. He is on warfarin variable and sotalol 40mg.He drinks wine and beer.
Today, Mr Dunbar presented with his daughter and he is moving with her. She will cook for him;hence, she requires health education regarding patient needs and monitoring. As per daughter, he is not comply with dietary alteration, exercise regime and medications, also he is ruluctant to adhere with that.
In view of the above, itbwould be appreciated if you could provide continuous monitoring and encouragement. Kindly, educate the daughter regarding the dietary guidlines.
