Re: Mr John Alex Frederick ,DOB:18 June 1987


I am writing on behalf of Mrs Norah John, wife of Mr John Alex Frederick who requires a home visit from your facility to improve his mobility .


On 12 July 2019, Mr Frederick had injury on the left knee following a fall while climbing downstairs due to severe headache .Daily dressing was done and a assistance was provided for daily living activities. Although, he can walk short distance ,he reported usual pain while walking.


Today , Mr Frederick has made good recovery. He was suggested to use walking sticks during amubulation.


Mr Frederick walks with limp and has unsteady balance after the episode of stroke three years ago.


In the view of the above, kindly provide Physiotherapy for improving his mobility.
