Dear Mrs David,
Re:Mr James Jung, DOB:10 April 1948
Iam reffering Mr Jung, who requires home visits weekly twice inorder to encourage him for occupational therapy and physiotherapy as he was diagnosed with spinal stenosis.
Mr Jung presented on4 January 2018 with severe back pain. Her radiological examination, revealed as spinal degeneration on L1-L2,L4-L5;therefore, the diagnosis was confirmed and was refferred for neurological assessment.In addition, physiotherapy was commenced; however,he refused for physiotherapy due to fear of pain.He also reported that he needs epidural steroid injection.
On subsequent days, Mr Jungshowed no improvements in pain which evidenced as difficulty in routine activities,grooming, showering and combing hair.He took more than recommended dosage as he insisted for higher and stronger doses of medications.
Mr Jung lives alone and is a retired factory worker. He has not seen his family for the last two days.He has had diabetes mellitus which was controlled with medications and has hypertansion,takes carvia 10mg twice daily.
It would be greatly appreciated, if you could arrange home visit for Mr Jung . It is imperative to monitor his medication as well as assistance to his daily activities. Kindly, encourage him to perform physiotherapy and to visit occupational therapst. Please arrange meals for Mr Jung.
