Dear nursing director,
Re: Ms Sydney loten,DOB: 14 August 1946
I am referring Ms loten,who underwent the colen polypectomy,requires ongoing assistance from your facility following her discharge today.
On 11 April 2020, Ms loten was admitted to the medical center for the routine colonoscopy. After the investigation,the multiple polyps have been noticed for which the surgery was performed. Postoperatively,the respiratory difficulties was found: therefore the arterial blood gas test was conducted,which revealed a metabolic acidosis consequently,she was transferred to the ICU for further monitoring. Subsequently,a good process was noted then she was shifted to the ward for observation. Subsequently,the excellent performance of the wound was founded.
However,she was ambulated by herself with a walker. Today, remarkable progress has been noticed.
Ms loten,Who is a widower lives with her adopted son. She drinks alcohol seldom but smokes 10-15 cigarettes per day for the past 35 years. She has been diagnosed with COPD since 2010, for which she is on 1 pratropium bromide inhaler 1 puff daily and formoteral 2 puffs twice daily. She has had diabetes and hypertension since 2007,2008 and is on tablet metformin 500 mg twice per day and tablet lasarton 40 mg once daily.
Pertaining to the above mentioned circumstances kindly provide continuing care at her home. Monitoring of her medication compliance as well as regular dressings change at her home would be profoundly beneficial. She has been scheduled for a follow up check up on 30 April 2020.
