Dear Ms Gill
Re: Ms Kylie Weiss,DOB:21 May 1952
I am referring Ms Weiss, who underwent an emergency angioplasty requires ongoing cardiac rehabilitation support following her discharge today.
On 07 July 2017, Ms Weiss was admitted to hospital with complaints of a jaw pain and a weakness of the both arms. She was brought to hospital by an ambulance. During hospital referral,an intravenous morphine 10mg and tablet aspirin 300mg was commenced. After the hospital assessment,an ECG and echocardiogram were performed which confirmed the my cardial infraction therefore the surgery was conducted. During hospitalization a blood test was done and the elevated blood cholesterol has been found consequently life style modification guidance was provided. During the hospital stay, she was reviewed by a physiotherapist and a dietition and the cardiac rehabilitation program as well as dietary modifications was discussed.
Ms Weiss, who lives with her husband is a pensioner and has a family history of the heart disease, she eats fried, fast foods,so her body weight was above normal.
Pertaining to the above mentioned situation, kindly provide continuing cardiac assessment about the compliance of medication loul cholesterol diet and regular exercise. An occupational therapist and a social worker review would be profoundly beneficial to improve her quality of life. Please note that, she was refrained from driving from the next 6 weeks.
If you require any further information, please do not hesitate to contact me at any time.
Yours sincerely
