RE: Mr John Alex Frederick, DOB: 28/06/1987

I am writing on behalf of Mrs Norah John, I am requesting a home visit for Mr John to help him regain his mobility. His movement is restricted after he sustained serious injuries to the left knee following a fall.


On June 2016, three years ago, Mr John had a stroke that has affected his speech and balance. He walks with a stick and he is mentally active. Please note that he lives with his wife, Mrs John who works as an auditor.


On 12/07/2011, Mr John woke up with serious headaches and subsequently fell down the stairs injuring his left knee. He was referred to our facility for pain management and wound dressing. In addition, he was supported by his wife to take care of his activities of daily living.


Mr John has made some remarkable improvements as his knee has healed and he can now walk with a stick.

It would be greatly appreciated if you could arrange a regular home visits to improve his mobility and kindly support him.
