Dear Ms Mary,

Re:Ms Jane Simms, aged 46

I am writing to update you regarding the current status of Ms Jane Simms, who is recovering from progressive multiple sclerosis. Your continuing monitoring and care would be greatly appreciated.

Ms Simms was admitted to the hospital on 26 July 2008 and was treated with Prothiaden 150 mg once a day along with her current medications. Her right buttock wound healed with proper regular dressing.

During hospitalisation, Ms Simms was provided with a low-calorie diet. She was initiated occupational therapy along with stretching and strengthening exercises. Gradually, she has shown remarkable overall progress.

It would be greatly appreciated if you could monitor skin integrity, weight and medication regime especially Prothiaden. It is necessary to encourage her social activities and continue an exercise program.

Please do not hesitate to contact me if you have any queries.

Yours sincerely,

Registered Nurse.
