Dear Mrs Adison,
Re: Mrs Margarat Adam, D.O.B:08 May 1945
Mrs Adam,a 75-year-old widow,who is recuperating from left total hip replacement surgery,requires home visits for 2 weeks.she is being discharged today.
Mrs Adam was admitted to hospital on 21 July 2021 following a fall at home while walking. Her x-ray, revealed fractured of left hip acetabulam,for which
the surgery was performed.Post-operatively, her pain wastreated with paracetamol injection, constipation was managed with dulcolax.In addition, along with her regular medication delirium and deltaparin were also commenced.Physiotherapy was initiated twice daily to enhance mobility and wound dressings were done on daily basis.
On subsequent days, Mrs adam can able to walk with stick without suppoort and does all her daily routine activities independently.She was advised to take deltaparin by herself with supervision.Overall, her post-operative recovery was uneventful and ready for disharge .
Mrs Adam is a retired school teacher and has no close relatives.Her neighbour is very supportive. She has had blurred vision and osteoporosis since 2018 and 2020 respectively.
Upon disharge, kindly monitor Mrs Adam medication and swelling.Please encourage her to perform exercise kindly asses her wound.Pleas note, her deltaparin to be dicontinued on 17 August and provide antiembolism
stockings on 06 September 2021.Of note, her medicine revival is on 22 September 2021.List of regular medications are enclosed with this letter.
