Ms Diana Forest, Luvanne physiotherapy centre, 588 livington street, South wales. 24-10-21 Dear Ms Diana Re: Mr: John Frederick 18th June 1987 Mr John Frederick had stroke 3years ago underwent emergency clot removal with a stent retriever was there in rehabilitation for a month, requires frequent home visits for physiotherapy. When met with an accident , he had complaints of extreme headache in the morning, was unable to climb stairs even, as a result he fell down from stairs and injured his left knee. He is able to walk small distances with the help from his wife Norah but complained of pain while walking. Needs continuous physiotherapy assistance to improve his mobility to meet his daily activities. Currently john is mentally active having mild slurring of speech having follow up with speech pathologist every 2 weeks. His GP suggested regular physiotherapy for improving his activity. His wife requesting physiotherapy assistance at home with regular home visits to improve her husband’s physical mobility. Please don’t hesitate to contact me for any queries.
