Dear Mrs Adison,


Re: Mrs Margarat Adam, aged 75


I am writing to request home visits for Mrs Adams for the next two two weeks. She has been recovering from left total hip replacement and ready for discharge today.


She is a widow and has no close relatives. However, her neighbour is very supportive. Her medical history is significant for osteoporosis and fractures.


On 21 July 2021, she fallen down at home while walking and was admitted to our facility. Hip X - ray was performed, which revealed fracture of left acetabulum. Therefore, the surgery was done after one day.


Postoperatively, she got delirium, which was recovered after five days. Alendronic acid and vitamin D were commenced along with paracetamol. Constipation was managed with dulcolax. In addition, deltaparin was self - administered by her; hence, supervision was required. Physiotherapy was initiated twice daily, and wound dressings were done on daily basis.


On consecutive days, she started walking with the help of walking frame, and could walk up and down from the bed. At present, she uses a stick for walking and able to perform all the activities of daily living.


Given the above, it would be greatly appreciated if you could visit her at 65 Old Dam Road Goondiwindi and monitor for medication compliance. Please note, she has slight swelling over the hip region, which needs to be observed. Exercise program should be continued. Cease deltaparin on 17 August 2021, redress the wound and provide antiembolism stockings on 6 September 2021. Appointment for medicinal review has been scheduled on 22 September 2021.
