Dear Ms. Bruin,

Re: Mr. Gerald Baker, aged 79

I am writing to inform you that Mr. Baker is being Discharged form city hospital today. He underwent a left total hip replacement. He is ready to discharge as he can walk using walker without difficulty.

Mr. Baker was recommended 100mg Aspirin daily post operatively also to take medicine for controlling hypertension, for pain relief Panadeine fort, Max 8 tablets per day, also needs daily dressing strengthening exercises ROM and stretching, occupational therapies for complete recovery sending walker and wedge pillow with the patient, also to heir a toilet raiser for 2 weeks.

During recovery period Mr. Baker was disoriented there is no history of Dementia it can be because of anesthesia. Hemoglobin level is low post operatively, blood transfusion 3units has been administered without complication. On discharge Hb is stable. Needs to monitor for signs of Anemia.

Mr. Baker needs to have his stables removed at city hospital on 21 September. Follow up blood test needs to conduct.
