Re: Phillip Satchell, DOB: 73 years old

I am writing to transfer of Mr. Phillip Satchell who has been taking care for Type 2 Diabetes Mellitus, Chronic ulcers on both the legs. Als suffering from Multiple sclerosis. He is moving to Maroubra stays alone as wife she passed in the year 2014. He is a small pensioner.

Phillip Satchell shares good relations with sons. They rarely come for visit as he is lonely and isolated needs proper support and care to cope up with current situation and improve social interaction.

Currently stable but this frequently deteriorates Phillip Satchell is on IV antibiotics OD daily and metformin for diabetes which is T.D.S Daily, for leg ulcers Recommends dressing twice a day for both the legs. Constant monitoring of blood sugar levels requested to refer to prince of Wales Diabetic clinic and to neurologist for MS Relapse.it would be appreciated if you could connect Phillip Satchell to support him to enhance his social network.
