Re: Mr Harley Chapman, aged 82
Mr Chapman, who is recuperating from a right below knee amputation (BKA), requires acute care, physiotherapy and further management following his discharge toady , and is being transferred to your facility.
On 08 April 2018, he was admitted with infected right diabetic foot wound and with symptoms of fever, chills along with formation of pus, abscess and gangrene at the site of wound;therefore, vascular surgeon recommended right below knee surgery. IV antibiotics were commenced for positive cultures.
Surgery was performed without any complications and the surgical site looked very clean, dry and intact . Antibiotics were changed from Iv to oral and opiates were prescribed for pain . Continuous monitoring for any signs of infection to be noted along with regular daily dressing at the site of surgery. Also encourage to take good nutrition and oral fluids. Assistance is required for mobilization and during his activities of daily living.
Mr Chapman has past medical history of type 2 diabetes, hypertension and other disorders for which he is dependent on medication and mobility since before the surgery.
On assessment after surgery, good progress was noted with no signs of infection and also with negative blood cultures, however his reduced mobility level is noted.
Based on the above mentioned details, continuing of antibiotics and pain medications is necessary along with physiotherapy. Mr Chapman will need to follow up with vascular surgeon, physiotherapy and occupational therapy after 2 and 4 weeks respectively.
