Dear Dr Stevenson,

Re: Mrs Fenna lisa,DOB:06 January 1964

I am referring Mrs Fenna Lisa, who has been diagnosed with grade 2 adenocarcinoma of the right breast with possible metastasis to the lumbar spine, requires a thorough evaluation and definite management at your service.

Mrs Lisa initially presented on 13 April 2009 with the complainant of a lump on the right breast .On examination, a cyst sized 1.5 cm was noticed. Her cytological examination revealed malignant cells, for which she underwent a biopsy. On subsequent visit, she was advised to contact the local breast cancer foundation for additional information regarding her care. Her hormone receptor was negative with no evidence of metastasis.

On today's visit, she presented with severe low back pain which radiates to back of the left leg. On assessment, pain and sensory loss at the anterior aspect of the thigh as well as absence of knee jerk was Exhibited. A weight loss of four kg also was detected.

Ms Lisa is married and has two children. She smokes heavily and consumes alcohol occasionally. Her menstrual cycles are regular and her menarche was at the age of 11.

Pertaining to the above details, it would be appreciated if you could perform chest and abdomen bone scan along with radiotherapy in order to alleviate the pain. Your expertise assessment and palliative care is profoundly beneficial.

If you require further information regarding Mrs Lisa, please do not hesitate to contact me.

Yours sincerely, Registered Nurse
