Re: Mrs Margaret Goldstein, aged 65
I am writing to return Mrs Margaret Goldstein to your facility who is recuperating from right elbow dislocation ,requires on going care and appropriate management at your service following her discharge today.
Mrs Goldstein was admitted to hospital on 08 November 2016 with the complainant of dislocated right elbow owing to a fall. During hospitalization, she has been commenced on Physiotherapy which needs to be continued after the discharge. Her right elbow has been supported with a cuffed arm sling. Although she has bladder incontinence, she is reluctant to wear disposable briefs. Currently she has made a good prognosis whereas, assistance is required in showering and incontinent care.
As you are aware, Mrs Goldstein has had hypertension and rapidly progressing early dementia for which she is on
Atacard Plus 32/12.5 25 mg and Remeron 15mg daily along with Oxycodone 5 mg three times daily as required. She uses wheel chair, eventhough she requires some assistance in transferring.
Pertaining to the above details, it would be appreciated if you could take over Mrs Goldstein and monitor her incontinence care as well as progress in the healing process of the elbow. It is essential to follow-up Physiotherapy and kindly note that her appointment has been scheduled on 28 November 2016 at 2pm at Eventide.
