Re: Akira Annaisha, aged 5
Akira and her sister require a follow-up on the vaccination program as Akira was diagnosed with acute meningoencephalitis due to complications of mumps. She is being discharged today.
About medical records, Akira and her sister had received vaccination at birth but, they are being lost. Additionally, her parents are unaware of measles, mumps and rubella vaccines. Subsequently, no compliance with the recommended vaccination was registered for both the children. Hence, your further advice would be greatly appreciated.
On 15th July 2019, Akira was admitted to the hospital with the above-mentioned diagnosis. Currently, her recovery is phenomenal.
In April 2019, Akira's family arrived in Australia as migrants from Japan. Even though, her father discerns spoken English for better communication an interpreter is required. Due to delays in the processing of migration documents, they do not have a family doctor.
Upon discharge, neurological check-ups for the next three months is imperative.
