Re:Ms Jamie Morgan
DOB:22 February 1956
I am writing to refer Ms Jamie Morgan who require education regarding low fat dietary guidelines and simple healthy recipes. He is being diagnosed with obstructive coronary artery disease. He underwent coronary artery bypass grafting.
Ms Morgan's medical history reveals ,she smokes 30 cigarettes per day as well as she drinks 300ml bottles beer three times a day.Her usual diet which she is maintained by eating sausages,deep fried chips,burgers,eggs and macdonalds.She is allergic to peanuts.
Post operatively, Ms Morgan is recuperating well,she was advised to reduced alcohol and to stop smoking.Her diet chart changed into low fat diet which she need to maintain now.Up until now,Ms Morgan's wound healed well where as now she is able to walk well. Since then there is a routine visit from social worker.
Upon discharge, Ms Morgan will have an appointment for followup visit to local GPDr stevenson George on 10 October 2017 at 2pm. Along with that,she has been advised to continue her exercise program with local physiotherapist.
It would be greatly appreciated,if you could provide education to Ms Morgan regarding low fat diet and some simple recipes to follow.
