Dear Sir/Madam,


I am writing to introduce Mr Alex Maydew who had neurological injuries, requires appropriate care, rehabilitation therapy and assistance in part-time online study. He is being transferred to your facility.


Mr Maydew had an accident 10months ago and he was in the coma stage for 8 months. Eventually, he woke from coma since two months and regained his normal brain functions. However, his speech is impaired. In addition, He is confined in a wheelchair and his mobility has been improving by physiotherapy.


Mr Maydew is depressed to be in an aged care setting which will affect his fast recovery. His mother also has same concern and requested to transfer him for better care. He can restart his study through part-time online classes and he can handle computers effectively. Previously, he was a third-year physical education student at south Australia university.


It would be greatly appreciated if you could arrange continuing care. Kindly provide ongoing speech therapy and physiotherapy. Please note the possible continuation of online education.
