Dear Dr Rodrigue,
Re: Ms Monica Ross,aged 26
I am referring Mr Monica Ross,who is recuperating from a suicidal attempt, requires detailed evaluation and thorough management at your service following her discharge tomorrow.
Ms Ross was presented to the hospital with the complainants of confusion, slurred speech, headache and diaphoresis secondary to consumption of paracetamol 20 tablets as well as one bottle of wine. On examination, although she responded verbally, she had weak pulse and low blood pressure. During hospitalization, she was administered with N acetylcysteine and laboratory investigations were done. Currently, she has made a good prognosis and tolerates orally; however, she looks pale and drowsy.
Ms Ross, works as a trainee accountant, is single and lives with her mother. Eventhough she consumes alcohol moderately, she is not interested in parties recently. Her hobbies are badminton and football but uninterested in those as well.
Pertaining to the above details, it would be appreciated if you could follow-up Ms Ross's care and monitor her condition accordingly. Kindly note, she is being discharged tomorrow in case her investigation reports are normal and subsequently, she requires review with a toxicologist.
