Dear Ms Gill,
Re: Ms Kylie Weiss, DOB:21 May 1952
Iam referring Ms Weiss who has been recuperating
from angioplasty,requires continued rehabilitative
care following her discharge today.
Ms Weiss was admitted on 26 July 2019 with history
of frequent pain and heaviness in both forearms.
On assessment,clammy skin and nauseated were noted.
Her electrocardiogram revealed ST elevation;therefore,
morphine 10 mg,aspirin 300mg and glytrin spray were
administered.
Ms Weiss repeat electrocardiogram indicted anterior
wall myocardial infarction.Consequently, the surgery
was performed. Her echocardiogram showed ejection
fraction of 35%. Currently,her cardiac status is
stable;however, she appears confused regarding
diagnosis probably due to reality of near death
experience.Cardiac exercise programme was initiated
by the physiotherapist.
Ms Weiss lives with her husband and works as a taxi
driver.She has history of dislipidemia and asthma
for the past 5 years.During hospital stay,
she was encouraged to reduce weight and cholestrol
levelby the dietician,as she is pbese for her age.
Kindly, educate Ms Weiss regarding risk factors of
myocardial infarction and its management.Please
arrange occupational therapist to provide instruction
of returning to work,driving and daily routine
activities.Kindly,encourage her to take adequate rest
for atleast 6weeks. Details of her medication is
enclosed with this letter.
