Dear Dietitian,
Re: Ms Patricia Cooper, aged 67
I am referring Ms Cooper, who has a history of aspiration, requires your expert assessment of her swallowing function and nutritional status.
On 21 March 2019, Ms Cooper had an episode of checking during breakfast. Consequently, she was distressed, coughed, and the obstructed piece of breakfast material was removed; therefore, she was returned to normal and no hospitalization was needed. She has had re-current chest infection, for which is on tablet keflex 500 mg fourth hourly for every week.
Ms Cooper, who is residing at our nursing home, has had type 2 diabetes mellitus and has a history of ischemic heart disease, for which she is on a nitroglycerin patch daily. She eats three full meals per day. Also, requests additional meal and snacks; therefore, she has been gained 8-kilogram weight for the past five months. She doesn't participate any social activities and doesn't smoke cigarettes, and drink alcohol.
Pertaining to the above-mentioned circumstances, kindly visit her at the nursing home and monitor the swallowing ability and dietary status. Proper dietary guidelines, the education about eating techniques would be profoundly beneficial.
If you require any additional information, please do not hesitate to contact me.
Yours sincerely,
Registered Nurse
