Re: Mr Martin Jones,aged 69 years
I am writing to request a home visit, to provide the prescription of syringe driver and pain relief medications.
Mr Jones,presented with the complaint of dysphagia in food and medicines and discharged to home with the regular assistance of palliative care.
During subsequent visits, noted to be severe ache and nothing by mouth, however; opposed to accept syringe driver. Nevertheless, at present willing to use a syringe driver.
It would be greatly appreciated,if you could do full assessment and provide appropriate management.
