Dear Mr Ortner,


Re: Mr Nick Daniel, aged 68


I am referring Mr Daniel, who underwent the hip replacement surgery, requires your expert instructions to regain his physical mobility and strength of his left hip following discharge today.


On 13 May 2019, Mr Daniel initially presented to hospital with a history of notch-up pain on the right hip and knee for the past five years. After the assessment, an x-ray was performed, which confirmed the right hip osteoarthritis; therefore, tablet indomethacin 50 mg thrice daily was commenced. After a month, he re-presented to the hospital with exacerbated right hip pain, for which tablet indomethacin 50 mg thrice daily was re-commenced. Subsequently, he was admitted to hospital with decreased movement of the right hip and declined muscle mass. In addition, he has been not responding to anti-inflammatory medication. Consequently, the operation was performed. Postoperatively, he was reviewed by a physiotherapist and has been initiated with the physiotherapy sessions.


Mr Daniel, who lives with his wife, is a retired farmer. He drinks alcohol and has had type 2 diabetes mellitus as well as has been allergic to penicillin.


In view of the above-mentioned circumstances, kindly provide advice for the improvement of his physical mobility. Education on the usage of a walker, weight-bearing exercises, and safely use of stairs would be profoundly beneficial.
