Re: Mr Paul Schroder, aged 45 years
I am writing to refer Mr Schroder, who has been diagnosed with type 2 diabetes, requires dietary management following the discharge today.
Mr Schroder, was presented with symptoms of fatigue and anxiety for 6 months.On examination,showed body mass index- 32 kg/m2, hba1c- 72 mmol/mol forwhich, he is on metformin 500 mg once daily. Counselling provided regarding blood sugar monitoring and hypoglycemia.
He is a single and lives alone.He has less contact with family and friends. In addition, he has a habit of smoking and drinking for long period along with this has a family history of type 2 diabetes on maternal side.
It would be highly appreciated, if you could encourage to quit smoking and drinking besides, you are guidance regarding dietary regimens are essential. Please note that, he has scheduled follow up for general assessment on 27 September 2018. Kindly contact Dr killing in case of emergency situation.
