Re: Mr Craig Hemsworth, aged 52
I am writing to update you regarding Mr Hemsworth, who is recuperating from tonic-clonic seizure, requires further assessment and management from your facility, following his discharge tomorrow.
On 19 August 2019, he was presented with bruises over left arm and left knee as well as bleeded tongue. He underwent an electroencephalogram and computed tomography, which revealed positive focal epileptic variants and subdural hematoma on the right occipital region. He was kept in intensive care unit for one day and neuro consultation was provided to him. Consequently, he was commenced on injection mannitol, ceftriaxone, phenytoin, tetanus along with tablets clipilate and amlodipine. In addition, input - output chart was maintained.
On the next day, he was shifted to the ward. Today, the ophthalmologist visited and advised him to give proper rest to the eyes. He was anxious about getting seizure after his discharge; therefore, he was educated along with his wife about managing the seizures at home.
In view of the above information, it would be greatly appreciated if you could continue tablet phenytoin 10 mg, once a day. Please note, he should revisit for review after two weeks. Please ensure that he use safety alert band, avoid driving and stress activities. He should consume healthy diet and have a proper sleep.
