Re: Mr Harley Chapman, aged 82 years


I am writing to transfer Mr Harley Chapman,who has been underwent right elbow knee amputation. He requires continuing care and physiotherapy, scheduled for the discharge today.


He was presented with fever and chills. On examination, infected wound was noted on right foot therefore, above mentioned surgery performed without complication. postoperatively, intravenous antibiotics was administered along with this regular daily dressing changes are provided. Surgical site has been closely monitored inorder to prevent infection and assisted with daily routine activities as well as ambulation. It reveals significant improvement in his recovery.


It would be greatly appreciated, if you could monitor medication compliance besides, he requires physiotherapy and occupational therapy after a month to improve circulation and mobility.


Please note that, he is scheduled for review with vascular surgeon in 2 weeks.


Mr Chapman is a retired postal worker and does not have interlinkage with the society for past six months.
