Re : Ms Joan Cummings, aged 81


I am writing to refer Ms Cummings, who is recovering from post falls anxiety syndrome, requires further care and assistance from your service following her discharge today.


She is widow and lives with her daughter, Amanda and son in law. Apart from that, she has had dementia and vertigo since 2008 and 2010 respectively. She had fracture of left neck of femur due to a fall in 2010.


She was admitted on 29 July 2011 with multiple bruises and postural hypertension. She underwent computed tomography of head and evaluated for medication. Ice packs were applied as needed. She was assessed for risk of falls, and commenced on exercise regime. She was advised to use hip protectors. She started mobilizing with the aid of wheelie - walker and manual support. She was provided assistance with activities of daily living. In addition, she was monitored for diet and fluids.

In view of above, it would be greatly appreciated if you could continue her physiotherapy program. Her current medication includes paracetamol 1gm, 4 to 6 hourly and oxycodone 5 to 10 mg, 6 hourly as needed, aricept 10mg, coloxyl and senna 1 to 2 tablets at night and caltrate plus 2 tablets in the morning. Please note, her follow - up appointments with the physician and optometrist have been fixed. Her daughter can be contacted at 0404994872.
