Re : Mrs Margaret

Goldstein, aged 61


I am writing to refer back Mrs goldstein, who is recovering from dislocated right elbow, requires further care and monitoring from your facility following her discharge today.


During hospitalization, she was commenced on atacand plus 32/12.5, 25 mg, remeron 15mg, and oxycodone 5 mg which needs to be continued, if required. She was advised to use the cuff with sling.


She has a slow healing process, however her skin is intact. Apart from that, she uses wheelchair for mobilization and requires assistance in transfer. She has incontinence of bladder.

She was suggested to wear disposable briefs, which she refused. Currently, she is medically stable and ready for discharge.


Given the above, it would be greatly appreciated if you could monitor her for healing progress and incontinence. Please note, follow - up appointment with the physiotherapist has been fixed on 28 November 2016, 2 pm at your aged care. Mr goldstein will collect her and transport back to your facility.
