Dear Ms Bates,
Re : Mr Benjamin, aged 63
I am writing to refer back Mr Benjamin, who is recuperating from total hip replacement, requires ongoing care and management from your facility following his discharge today.
Post-operatively on 5 September 2009, he appeared disoriented, possibly due to anaesthesia. His haemoglobin level was dropped by 7.1 gm, therefore three units of packed red blood cells were transfused. Apart from that, wound dressings were changed on daily basis. He was commenced on stretching exercises.
Currently, he has attained a good recovery. His haemoglobin level is 11.2 gm, and he walks perfectly with the help of wheelie - walker.
In view of above, it would be greatly appreciated if you could provide panadeine forte 6 tablets per day, and continue the exercise program. Please note, wheelie - walker and wedge pillow have been provided by the hospital and toilet raiser should be arranged for next two weeks from the local medical supplier.
