Dear Supervisor,
Re: Mrs Margaret Goldstein, aged 61
I am writing to transfer back Mrs Goldstein who requires ongoing care for her dislocated elbow at your facility following her discharge today.
On 08 November 2016, Mrs Goldstein was admitted following a fall at her aged Care home and she was diagnosed with dislocated right elbow with skin intact.
During hospitalization , Mrs Goldstein's elbow is supported by cuff with sling. Thus, gradually injury is getting healed. She uses wheelchair for mobility and requires assistance for transfer and shower. In addition, she has bladder incontinence but reluctant to wear disposable briefs from yesterday. She was commenced on atacand plus, 32/12.5 twice daily, remeron, 15mg once in a day and oxycodone ,5mg as required.
In view of the above mentioned information it would be greatly appreciated if you could monitor her healing process of elbow and incontinence care. Kindly , assist with her in hygienic needs and incontinent care. Of note, she has appointment on 28 November 2016, at 2pm in your facility with physiotherapist.
