Dear Ms. Bruin,

Re: Mr. Gerald Baker, aged 79

I am writing to update you regarding the health status of Mr. Baker, who has been under our care for total hip replacement. He is being discharged today and requires continuing care from your facility.

As you are aware that, Mr. Baker was admitted to this hospital on 03 September, 2018 and underwent the surgery. Post-operatively, he was disoriented due to the side effect of anesthesia. He was transfused 3 units of packed red blood cells, as his HB level dropped.

Mr. Baker was recommenced on Aspirin 100mg daily as he has hypertension. He was commenced on Panadeine Forte and dressing were performed to preserve the skin integrity. He is also undergone a serious range of motion, stretching and strengthening exercises. In addition, occupational therapy were performed. Currently, he has made good recovery.

It would be greatly appreciated, if you could monitor medications especially Panadeine Forte. Please continue his exercise programs. Kindly monitor his Hemoglobin level. Kindly note that our hospital providing wedge pillow and walker. And it is necessary to hire a toilet raiser for 2 weeks.
