Re: Mr Jamie Morgan, D.O.B:22 February 1956
I am writing on behalf of Mr Morgan, who requires guidance on dietary regimen in low fat diet.He has been underwent coronary artery bypass grafts following the diagnosis of of obstructive coronary artery disease, scheduled for discharge.
He consumes 30 cigarettes and 3 bottles of beers each day. He prefers stodgy foods such as sausages, deep fried chips, burgers, eggs, MacDonald's for which, body mass index revealed moderately obese.(height- 177 cm and weight- 96 kg) Consequently, underwent coronary artery bypass raps thrice earlier.He is single and lives alone in his own house, he works as a fencing contractor.
During hospitalization,health education has been provided about cessation of smoking and minimise of alcohol consumption.
In the above-mentioned circumstances, provide a dietary guidance and lifestyle modification.
